A holodeck program by Tuvok becomes dangerous.

Crew members discover an unfinished holodeck program, Insurrection Alpha, in which the Maquis on board mutiny against the Starfleet officers, and it becomes very popular. Tuvok admits that he wrote it early in the voyage as a security exercise in case of a Maquis uprising, and abandoned it when the two crews came together. Tom Paris offers to help him finish it as a proper holonovel.
When they begin, the program traps them inside. The late Seska, who had found Tuvok's work before she left Voyager, had rewritten it as a trap, and her changes spread into the ship's other systems. With Janeway and Torres trying to help from outside, Tuvok sets off an overloaded phaser that shuts the simulation down. The third-season finale is a self-aware story about storytelling, and it brings Seska back one last time in holographic form.
